Estradiol on TRT without an aromatase inhibitor
Why the ratio matters more than the number, what high and crashed estradiol feel like, and the order people try things in.
Estradiol is the estrogen men make from testosterone. On TRT it rises with the dose, and that is mostly good: it protects bone, joints, HDL, mood and libido. The mistake most people make in year one is treating the estradiol number like a problem to fix.
Read the ratio, not the number
Take testosterone in ng/dL and divide by estradiol in pg/mL. Around 15 to 30 is where most men feel best. A testosterone of 900 with an estradiol of 45 is a ratio of 20: fine, even though 45 is above some lab ranges. A testosterone of 400 with an estradiol of 45 is a ratio of 9: that is a lot of aromatising for the dose.
Ask for the sensitive (LC-MS/MS) estradiol assay. The standard immunoassay reads men's levels poorly and produces false highs.
What high and low feel like
- High: water retention, puffy face, sore or puffy nipples, moodiness, emotional over-reaction.
- Crashed (too low): dry, aching joints, flat mood, no libido, poor erections, fatigue, sometimes anxiety.
Crashed feels worse than high, lasts longer, and is almost always caused by an aromatase inhibitor.
What people try, in order
- Split the dose. Big infrequent shots aromatise at the peak. The same weekly dose in two or three injections often brings estradiol down and the ratio into place with no other change.
- Body fat. Aromatase lives in fat. Leaner men run lower estradiol on the same dose. This is slow but real.
- Alcohol. It raises estradiol for days. Skip it before a draw and notice whether the symptoms track your weekends.
- Soft options. DIM and calcium-D-glucarate are what people reach for before a drug. Evidence is thin; some feel a difference.
- An aromatase inhibitor, last. A quarter of a tablet, once or twice a week, only for clear symptoms that the steps above did not fix. Retest in three to four weeks. If joints ache and libido drops, that is crashed estradiol: stop.
When the number is low without an AI
Very low body fat, a hard diet, or a low dose. Estradiol follows testosterone; a low dose with a low estradiol usually means the dose is low, not that the estradiol needs raising.
Keep it on one line
Estradiol swings with timing and assay more than most markers. Same lab, same draw timing, and a line showing every result next to the testosterone it came with is the only way to know whether a change did anything.
